Innovative Services Clairemont Villa Adult Day Health Center provides a structured day program that promotes socialization and delivers skilled services to support senior citizens and adults with disabilities. Each participant is provided an individualized plan of care, which may include any of the following required services:

Nursing Services:

  • Monitoring of vital signs
  • Medication administration and management
  • Emergency medical services
  • Diabetic care, education and insuline administration
  • Wound care
  • Ostomy and catheter care
  • Direct communication with physicians,
    hospitals, clinics and pharmacies

Physical/Occupational Therapy Services:

  • Strengthening, conditioning, improving balance, normalizing ambulation and stabilizing gait
  • Assistance with improving gross and fine motor coordination
  • Restoring and maintaining maximum function
  • Assistance with overcoming challenges related to Activities of Daily Living such as transferring, ambulation, upper and lower body dressing, grooming and hygiene, and self-feeding

Medical Social Worker Services:

  • Counceling and emotional support
  • Behavior monitoring and management
  • Referrals to community based programs and services
  • Assistance with completion of Medi-cal HMO applications

Psychological/Couseling Services:

  • Psychotherapy and behavior modification
  • Counceling and behavior management
  • Emotional support
  • Alternative to psychotropic medication administration

Program Aide:

  • Maintenance exercise programs
  • Assistance with transferring, ambulation, personal hygiene, feeding, toileting and incontinence
  • Helping to engage in meaningful therapeutic activities programs of choice

Transportation Services:

  • We provide full transportation from your doorstep to the center and back to the home or boarding care. Our buses has the ability to pick up the participants with wheelchairs.
  • ADA/MTS application and scheduling


  • Contributes to the health and well-being of its participants and their families
  • Is a viable alternative to help delay or prevent hospitalization
  • Assists with restoration of maximum level of function
  • Provides health monitoring, social stimulation, nutritionally balanced meals and therapeutic activities
  • Respite for family members who are trying to balance caring for their loved ones, work and personal life
  • Provides an environment that is "Happy, Safe and Comfortable"
